Order your Replacement card or CNA Course Completion Certificate.  Only cards or certificates issued by Southeastern School of Health Sciences(SSHS) can be replaced by SSHS.  

• After we receive your order we will verify the name with our data.  
• Orders will be processed within 3 business days.  
• The replacement document will be EMAILED to the email address provided on your order.
• If we are unable to verify your information you will be contacted via email for additional information.  
• Email info@southeasternschool.org if you have questions regarding your replacement card.

All requests must include your full name, location of course, and the approximate date you took your course in the comments section.
